Most of the form is designed in a similar manner while there are some unique features available in each form. In this article the process to create main body of the form is explained with the example of a Exit Interview Form.
Question Types : the following types of questions can be designed

6. Multiple choice
7. Multiple choice grid
Each of the question types is explained below with an example. Wherever text based response or comments are taken, maximum of 8000 characters can be inputted.
1. Text based
As shown above you can create a question through which you can take text based inputs. If you select "Response Compulsory" as yes then the respondent will have to provide response to at least one option.
2. Score based
3. Ranking
If you want the respondent to give a numeric score out of a maximum score then
you can use this option. As shown above you can decide
- what is the maximum score for each option
- if response is made compulsory then score of all the options will have to be
provided
- if comments are needed and are compulsory
4. Single choice
If you wan to capture a unique response from a set of predefined options then use this question type.
5. Single choice grid
If you want to capture response on a scale across multiple sub parameters then
you can use Single choice grid. By default only one options can be selected along
a row (X Axis). This question type has two sub options
a) Only one response per column - in this case the respondent will be allowed
to select only one option along the column (Y Axis)
b) Multiple responses per column - in this case the respondent will be allowed
to select multiple options along the column (Y Axis)
The responses will be captured as a radio button selection.
6. Multiple choice
If you want to capture multiple choice based response to a question then this question type can be used. You can additionally use the parameter "Choices to be captured" to control how many parameters the respondent has to provide. If this parameter is not selected or if < operator is selected then the response is optional.
7. Multiple choice grid
If you want to capture multiple responses across multiple parameters then you
can use Multiple choice grid. The responses can be captured in two ways
a) Checkbox - in this case the respondent will be allowed to select multiple
options along the column (Y Axis) through a checkbox
b) Dropdown - in this case the respondent will be allowed to select multiple
options along the column (Y Axis) and will be able to rank them using a dropdown,
the values in the dropdown can be defined in setup
