Design of Performance Sheet

Performance Management sheet is a key form which gets updated throughout the year and collates all the aspects of employee’s performance.

A sample performance sheet is shown and explained in this article but the sheet you might be seeing would be different as the design of the format is highly customizable and sections / fields which are visible here might not be used in your company.

1. Header

Header shows basic details about the employee and names of assessors and reviewer.


2. Performance Review Summary tab

Performance goals are captured in this tab in the beginning of the process and towards the end of the process the final comments and ratings are also shown on this tab. The option to add new criteria, delete an existing criteria, change weightage can be given to employees, assessor or reviewer. It is always available with the Performance Manager.


3. Periodic Review tab

Inputs of periodic self assessment and reviews can be done by the employee and their assessors / reviewers on these tabs. Each criteria is shown in the table and employees / assessors / reviewers can use the Edit button to fill up the details against the criterion.

4. Individual Development Plan

Development goals and recommended trainings are identified during the annual review period and captured in this tab.

This tab is visible only during the annual review stage and is not visible to the employee. The assessor / reviewer can update the skill gaps and identify trainings which need to be completed. Refer article for more details.

5. Career Development Plan


This tab is visible only during the annual review stage and is not visible to the employee. The assessor / reviewer can provide inputs which are used for employee's career planning. Refer article for more details.

6. Key Behavior Assessment

If Key Behavior Assessment feature is activated then during the Annual Review stage employee / reviewer and assessor are required to score and give their feedback on the demonstrated behaviors of the employee.


7. Final Feedback and Final Rating


When the annual review is over the final rating and summary feedback is given for the employee's performance. This information is visible on the Performance Review Summary tab.

8. Profile View

If the education details, work experience of an employee is to be viewed click on "View Profile" button on the bottom of the page. Details as shown below will be visible in a popup.
