My Payroll Reports - Form 12BA

This report is to download form 12BA Forms. Form 12BA is required to file at the time ITR return. If any perquisite is part of your annual salary then you can get details from this form.


Click on generate report and then click on "Export To PDF"  to get 12BA form in PDF format.
