The option to fill up various nomination forms and declarations associated with Payroll is available to employees and also to the Payroll Manager who can do it on their behalf.
There are five forms in total but all might now be visible (form visibility is linked to feature activation), for example - if employee is not participating in ESI then ESI nomination form will not be visible. Sample forms are attached with the article.
1. Gratuity Nomination
To submit the gratuity nomination provide the name, relationship, date of birth
and the percentage to be given for each of the nominee. Numbers of nominee can
be more than one but % of Gratuity should be equal to 100 across all nominees.
Once the nomination form is generated it can be downloaded using the Download button or Cancelled using the Cancel button. The Payroll Manager can file the nomination form with the authorities and once it has been filed he can update the status as Filed from his page. The employee would then see the status as Filed.
Once the nomination has been filed then only the Payroll Manager can cancel it.

2. PF Declaration - Form 11
Some part of PF declaration data is captured as a part of the new joiner information.
The remaining needs to be updated as per the form below.
3. PF Nomination - Form 2
This form is used for the purpose of declaring nominations for EPF and EPS.
Fill up the data as required by the form and click on "Generate" button, the form is generated and you can download it.
Once the nomination form is generated it can be downloaded using the Download button or Cancelled using the Cancel button. The Payroll Manager can file the nomination form with the authorities and once it has been filed he can update the status as Filed from his page. The employee would then see the status as Filed.
Once the nomination has been filed then only the Payroll Manager can cancel it.
4. ESI Nomination
This form used to capture the nomination details of the insured person for ESI.
Fill up the data as required by the form and click on "Generate" button, the form is generated and you can download it.
Once the nomination form is generated it can be downloaded using the Download button or Cancelled using the Cancel button. The Payroll Manager can file the nomination form with the authorities and once it has been filed he can update the status as Filed from his page. The employee would then see the status as Filed.
Once the nomination has been filed then only the Payroll Manager can cancel it.
